Welcome to Fork Lane’s Health Office!

My name is Andrea Mazz and I’m the Fork Lane School Nurse. If you have any questions/concerns please don’t hesitate to call me. I wish everyone a safe and happy school year.

Here you will find some important information. 

Attendance: 
If your child will be absent from school please call (516) 733-2345. 
Please state your child’s name, spelling of your last name, teacher, grade and reason for absence.

Medications:
In order for any medication to be taken in school please see the link for the Hicksville Public Schools form.

Physicals:
are required for New Entrants, and students in Grades K, 1, 3, and 5th grade. The NEW form that needs to be filled out is at the following link.
